Boarding Form

* VACCINATIONS MUST BE CURRENT FOR ALL BOARDING PETS *

For the protection of your pet, we require that your pet be current for the following:

Dogs: DA2PPC, Bordetella (kennel cough), Rabies, Influenza and fecal examination
Cats: FVRCP, FeLV and Rabies

If my pet's vaccinations were given anywhere other than AMSH, I authorize the release of my records. I understand that I am responsible for providing the history of vaccinations for my pet before or upon presentation of boarding. If proof of vaccination is not provided, I authorize AMSH to vaccinate my pet while boarding at my expense. If my pet has fleas or ticks, I understand that AMSH will treat at my expense. (Proof of vaccination must be presented within 3 hours of arrival)
