That’s why the team at Animal Medical and Surgical Hospital in Tulsa and Broken Arrow is committed to making the process as smooth, transparent, and safe as possible. Our Surgical Information Form and Pre-Anesthetic Packet are designed to guide you through every step of your pet’s upcoming procedure, giving you peace of mind from start to finish.
What to Expect on Surgery Day
On the day of your pet’s scheduled surgery, we’ll collect your completed Surgical Information Form and any other required documents. Our team will walk you through the process, answer your questions, and ensure all necessary authorizations are in place before proceeding.
If you’re a new client, please note that a pre-surgical exam may be scheduled before the procedure. At drop-off, our veterinary nurses will help your pet get settled and prepare them for surgery in our designated surgical area.
Why This Form Is Important
This form helps us:
- Ensure we have the correct contact and medical history information.
- Obtain consent for the procedure and any optional services (like pre-anesthetic bloodwork).
- Review and confirm your understanding of anesthesia and surgical risks.
- Keep you informed and reachable during your pet’s procedure.
We ask that all clients provide reliable phone numbers on the form so we can reach you if needed during the procedure. If we are unable to contact you in the event of a decision-making need, your pet’s surgery may be delayed.
How to Prepare for Surgery
Before your pet’s procedure, please:
- Withhold food and treats after 9:00 PM the night before surgery.
- Water is allowed overnight but should be removed in the morning.
- Skip any morning medications unless otherwise directed by your veterinarian.
Please arrive on time the morning of your pet’s surgery. At pickup, a member of our team will provide you with written discharge instructions and review them with you to ensure your pet’s smooth recovery at home.
